Your name, address, and phone number;
A statement that you consent to the jurisdiction of the federal court in your district and will accept service of process from the person who submitted the original DMCA notice.
We will forward any valid counter-notice to the original complainant and may restore the content unless the complainant files a court action within 10 business days.
